Patient Family Advisory Council (PFAC) Positions Available |

Do you know a patient who would be enthusiastic about supporting and improving care at FHN? Who is able to maintain an open and caring mind, sharing both positive and negative experiences in a constructive way? Encourage them to join our Patient Family Advisory Council (PFAC) and help FHN improve on our commitment to provide a safe, compassionate, and innovative healthcare for our community.
PFAC is a team of 10 diverse community members that work in partnership with FHN leaders to provide feedback on how FHN patients understand and navigate healthcare in the modern age. In learning from the patient and/or family perspective, we will continue in our focus to improve the patient experience, increase safety, and enhance the quality of care throughout the organization.

Freeport Memorial School of Nursing Alumni Scholarships Available |

The Freeport Memorial School of Nursing Alumni organization is offering scholarships to help nurses and nursing students further their education.
“The idea was proposed at our annual banquet in 1990 by Phyllis Almberg,” said FHN Community Services Coordinator Jackie Roach, RN. “Three volunteers investigated the idea and in 1991 we voted to establish a scholarship program. We were proud to begin the program by awarding three scholarships in 1993,” Roach said. “Since then, we’ve awarded 83 scholarships ranging from $500 to $1,000 each.”
The scholarships are available to help cover the cost of pursuing a career in nursing for those just starting out or for nurses pursuing advanced nursing education. Applications are available through area high school guidance counselors, at FHN Memorial Hospital in Freeport, at SSM Health Monroe Hospital and at the Highland Community College Foundation office.
The organization’s selection committee determines scholarship recipients based on educational and vocational goals, extracurricular and community activities including organizational, civic, or volunteer work and honors, awards or recognition applicants have received. Applicants must be accepted in a nursing program or pursuing advanced nursing education. The deadline for applications is June 1. Scholarship recipients will be announced at the 2023 annual banquet June 10. Scholarship funds are sent to the recipients’ school in their name.

Vizient Employee Discounts
To register for a free account with Abenity, enter “FHN” in the organization name field, and you’ll be directed to the Abenity dashboard. Navigate to the employee discount/perk that you’re looking for using the website’s menu system or search bar. You can also download the free, convenient Abenity app via the Google Play Store or the Apple App Store. Then follow the steps provided to access the discount.
• At the bottom of the main Abenity dashboard page, there are ways to look up categories of products, how-to videos, etc.
Clicking on “New” or “Popular” will give you the list of products that people often ask about (rental cars, electronics, etc.)
• Abenity has discounts for zoos, amusement parks, museums, etc. in IL and elsewhere
• Neither Vizient nor FHN can guarantee that these employee discounts are the best deals available. You may be able to shop around for better deals via Groupon or other programs.
Example: Cellphone Discounts
Verizon Wireless
• Existing Verizon customers should look for the heading: “Already have a Verizon Wireless account?” and click the small arrow ( > ) underneath.
Sign into your Verizon mobile account and follow the steps provided. For assistance, call the Verizon wireless employee validation center at 800-890-8007.
• New Verizon customers should go to a Verizon store to create an account and choose a plan or call the new account hotline at 800-922-0204.
AT&T Wireless
• Existing AT&T customers should log into their account and follow the steps provided. For assistance, call the AT&T wireless business division at 800-331-0500.
• New AT&T customers should go to an AT&T store to create an account and choose a plan (or call the new account hotline at 866-945-8251.) (NOTE: There is an option to print a coupon to bring to an AT&T store, but this will not work in Illinois, Wisconsin, or Iowa)
You will need to provide them with Foundation Account Number (FAN #): 02562984.
US Cellular
FHN employees are eligible for a Partner Employee Discount (PED) through US Cellular. (This offer is not through Vizient or Abenity).
The discount is up to 15% off on qualified plans. See a US Cellular associate for more details. If requested, please provide the US Cellular representative with this PED code: PED4551. You will also be asked to verify your employment with FHN, which you can do by showing a paycheck stub or an employee name badge, and show a valid driver’s license or state ID.
Brown's Shoe Fit
FHN employees are eligible for a 15 percent discount on shoes from Brown's Shoe Fit in Freeport when you present your employee name badge. Payroll deduction is also available.
Sherwin Williams
FHN employees are eligible for a 15 percent discount on household paints and stains, as well as brushes and rollers.
Family YMCA of Northwest Illinois
FHN employees get 15 percent off all membership categories with no joiner fee or contract required, a free 7-week group swim session for employee's child, one free personal training consultation and health and wellness orientation, plus free land and water fitness classes. Free childcare available to all YMCA members while working out or swimming.
Butitta Brothers Automotive (Freeport)
FHN employees are eligible for a 10 percent discount on any service over $100, and will do scheduled pickup/delivery of staff vehicles while at work.
Advantage Marketplace

Indigenous peoples across North America celebrate the April – May Iroquois Strawberry Season in April and May. It is a time of thanksgiving and performing the Great Feather Dance.
Canada celebrates Asian Heritage Month in May. The Senate of Canada passed
a motion in December 2001 designating May as Asian Heritage Month. Canadians take part in festivities commemorating the legacy of Asian Canadians and their many contributions which have helped Canada become the multicultural and diverse nation it is today.
The United States also celebrates May as Asian Heritage Month. Generations of Asian Americans and Pacific Islanders have strengthened the U.S. as a nation. Many ethnic groups worked tirelessly to build a national railroad, paving the way for western expansion. The first Asian/Pacific Heritage Week was celebrated in 1979, in response to little recognition of this population during the 1976 bicentennial. By 1990, the celebration was one month long and then made official in 1992.
May is National Older Peoples Month in the United States. It was established by presidential proclamation in 1963. The entire nation is encouraged to pay tribute in some way to older persons in their communities. It is celebrated across the country through ceremonies and events.
Both Canada and the United States celebrate Jewish Heritage Month in May. The event provides an opportunity to reflect on and celebrate the incredible contributions that Jewish people have made to the country and in communities across North America.
May is the time of the May Hopi Kachina Dances for Indigenous people across North America. During late spring and early summer, long lines of Kachina Dancers representing many spirit-powers perform dances in Hopi pueblos.
Today (May 18), Christians commemorate the ascension of Jesus into heaven with Ascension.
Canada celebrates Victoria Day on May 22. Queen Victoria’s birthday (May 24, 1837 – 1901) was declared a holiday by the Legislature of the Province of Canada in 1845. The day is also celebrated in Quebec as the Day of the Patriots, honoring the French patriots of 1837 – 1838 for their contribution to freedom and national recognition.
People of the Baha’i’ faith celebrate the Declaration of Bab on May 24. The religion was begun in Persia (now Iran) on this day in 1844, by the prophet Bab, who announced his mission as the founder.
Canada observes Indigenous Awareness Week beginning May 24. First introduced in 1992, it is held on the four days that follow the Victoria Day long weekend. It was designed to increase public awareness of Aboriginal peoples among federal public service employees.
